If you are ready to get started, we welcome patients to use our patient portal to complete all forms needed for upcoming visits to Boothbay Family Dentistry, as well as access times and dates for upcoming appointments. You can access the patient portal by clicking HERE. To help limit the spread of all bacteria, germs and viruses, we encourage you to fill out all forms before your visit. Currently, we are not accepting payments through the portal.
If you have not set up your patient portal, please call us so we can send a secure link inviting you to create your password. To verify your identity, you will be asked to provide your birthdate and zip code. If you are a seasonal resident, the system will require your summer zip code. This link will expire after 24 hours. If needed, please give us a call and we will be happy to send you another one. Per your preference, each patient can have their own unique portal login/password, or we can group a household together into one login/password. Please let us know if you would like your portal changed from its current setup.
After creating your portal, the link in the sign-up email will no longer work. Please go to clarkson.curveconnex.com to access the portal after your password has been created.
